Nomination Form
Alumni award nominations are accepted year-round. Those received by 11:59 p.m. on May 31 will be reviewed for the same year's awards program. Those received after May 31 will be reviewed for the following year's program.
A CV/resume and at least one letter of support are required with every nomination. A maximum up to three letters of support are allowed. Historically, nominees with more supporting documents receive higher scores.
Only one Distinguished Alumni Award will be conferred each year.
