Judith Lloyd Storfjell Distinguished Award for Scholarly Practice

The UIC College of Nursing Judith Lloyd Storfjell Distinguished Award for Scholarly Practice is an annual award to recognize a UIC College of Nursing faculty member who has made an outstanding contribution to nursing practice.

Eligibility

UIC Nursing faculty or adjunct faculty

Deadline

Submissions are due each year by May 1

Nomination Process

Nomination should include the following information:

1.) The nomination form

2.) Two letters of support from peers documenting:

  • Evidence of outstanding contributions to nursing practice including:
    1. Program development and/or practice that has changed lives and improved health of individuals and communities;
    2. Advanced best practices resulting in improved health outcomes;
    3. Influenced Nursing practice through publication, presentations, political action, and/or the media;
    4. Demonstration of a commitment to the education and mentorship or future nurse leaders.

3.) Nominee’s recent Curriculum Vitae.
