UIC Nursing faculty or adjunct faculty

Submissions are due each year by May 1
Nomination Process
Nomination should include the following information:
1.) The nomination form
2.) Two letters of support from peers documenting:
- Evidence of outstanding contributions to nursing practice including:
- Program development and/or practice that has changed lives and improved health of individuals and communities;
- Advanced best practices resulting in improved health outcomes;
- Influenced Nursing practice through publication, presentations, political action, and/or the media;
- Demonstration of a commitment to the education and mentorship or future nurse leaders.
3.) Nominee’s recent Curriculum Vitae.
